As the 2022-23 school year comes to an end, we turn our attention to the 2023-24 school year.  In order for your son/daughter to participate in athletics next year,  please review the following checklist.

Online Registration Through Student Central (old Planet High School):

Returning athletes entering 8^th through 12 grade:  

After 6/1/23, you and your son/daughter will need to log into your Student Central accounts and update the forms: https://www.planeths.com/?keyword=/user/signin&schoolID=0  You do not need to create a new account for yourself or your son/daughter. If you have any questions or issues, please contact customer support at 866-448-9438 or support@bigteams.com.  Once all the forms are updated your student-athlete will need to get a new 2023-24 athletic physical. Physicals for the 2023-24 school year must be done on or after 6/1/23.  Any physical done prior to this date is acceptable as long as the medical professional who performed the exam, reviews the physical then signs and dates the new physical form with the original physical date and the date in which he/she reviewed it.

Athletes entering 7^th grade or new student-athletes to QVSD entering 8^th through 12^th grade

QV athletics has gone paperless for most, if not all paperwork (physicals, medical release forms, zero tolerance forms, etc) required to participate in athletics at QV.  Each parent/guardian and student-athlete must create an account on https://www.planeths.com/?keyword=/user/signin&schoolID=0.  Once both the student-athlete and their parent/guardian have created an account, you will link the accounts together and all the required documents will be there to complete.  All signatures will be done electronically.  The PIAA physical packet will be filled out in the student-athlete’s account and the actual physical form that the doctor/approved medical provider fills out will then be uploaded by you. Physicals for the 2023-24 school year must be done on or after 6/1/23.  Any physical done prior to this date is acceptable as long as the medical professional who performed the exam, reviews the physical then signs and dates the new physical form with the original physical date and the date in which he/she reviewed it.

Free athletic physicals are being offered by Greater Pittsburgh Orthopedics and Greater Pittsburgh Physical Therapy on Monday June 5^th.  These physicals are only for student-athletes entering 9^th through 12^th grade.  For information on how to sign up for a physical please click here: https://qvquakers.org/main/otherad/contentID/55408219

Regardless of where your son/daughter gets their physical completed, you will need to print and fill out Section 5 of the PIAA Physical Packet (Health History Form) and Section 6 (PIAA Comprehensive initial pre-participation physical evaluation and certification of authorized medical examiner form) and take those with you to the physical appointment  Once you have a signed and dated Section 6 completed for your son/daughter, that form needs to be uploaded to their PlanetHS account.  Remember the form must be signed and dated by the appropriate medical professional on or after 6/1/23.  Blank copies of the Section 5 and 6 forms can be found in the middle of the body of information about the athletic physicals here.

In order for your son/daughter to be eligible to participate in a sport at QV, all their forms must be completed and electronically signed by the parent/guardian and the student-athlete.  Failure to have all forms and signatures completed will result in your son/daughter not being cleared to participate.  

Baseline ImPACT Testing:
